Your concern brings up a quandary I would have. My wife is a partner in a small town medical practice. Her leaving immediately would be a huge burden for the other docs in her practice(the five of them cover the ER 24/7, 365 days a year) so we would have to consider other options. Her agreement is that she is required to give a 6 month notice or be penalized $1000/day. I guess $180,000 wouldn't be a huge issue if we had $100+ million, but there is an ethical quandary. I guess she would continue until we could arrange a replacement or offer to pay partial costs of a locum tenens for the 6 months.
As for what to do with the money. First of all it gets split in 1/2 with my best friend of 30+ years. I think the first thing he would do is give his wife half and get divorced. After 3 marriages he may realize he is not the marrying type.
Then with our half my wife and I would split the ticket with our 5 siblings and 4 or 5 close friends and other relatives. I envision we would give them 2-5 million each. No fun having money if the people I like and like me(not many of them) don't have any.
We would then build a modest(modest for a multimillionaire) home on my wife's family property in rural northern Wisconsin. This is a place with dirt roads, poor cell coverage, wells and septic tanks. Then buy up all her grandfathers old farm land that was sold off 25-20 years ago. Probably about 700-800 acres total.
Next would be the winter getaway in the Caribbean. Probably Puerto Rico as that is where my father is from.
Lions share of the money will then be invested so we can live off of interest. May start or buy a business or two. Nothing that will require me to have day to day control.
Finally I would take a chunk and set up a family trust. The purpose of this is twofold. First it will be a vehicle to make charitable contributions now and going forward. Secondly, it will be a means to take care of the next and future generations of the family. When it is time for college or tech school the family trust can pay for or assist. The trust can help with mortgages and other large purchases. The trust will allow for the following generations to get a leg up but not a life of leisure.

Not guaranteed. What if 5 or more people hit the #'s?
What if you missed a handful of #'s and the winning # was in the handful?
