Born and raised in western Pennsylvania, went to college in northeastern Ohio.
My father was a pharmacist and the family business was a small independent pharmacy in a neighboring rural town. The low income people who got their prescriptions paid for through Medicaid would come in to get lice medication, and that's the first I heard of such a thing.
From what I understand, head-to-head close contact is the most common means of transmission.

Well, no, I just told you that it wasn't common in my neck of the woods.
I only ever learned about it by a few people coming into my father's store (in a neighboring rural town) and getting their prescriptions filled.
