Many recipes for caramelised onions do require the addition of sugar but there are naturally occurring sugars in onions as well. So it isn't really incorrect to describe onions cooked without additional sugar as caramelised provided they are cooked in such a way that the natural sugars do caramelise.

What amuses me (rather than annoys me) is people who pronounce Byron's Don Juan as if it were the Spanish (Don H-won), as opposed to the correct pronounciation of Don Jew-on.
How can you be sure it is Byron's Don Juan they are talking about rather than Tirso de Molina's or Moliere's?
I believe Gary Trudeau had a Doonesbury cartoon on this phenomenon. He reckoned it was your second option; it was done to produce the appearance of sincerity, as if one habitually lies to all other callers but in your case is telling the truth.
